Community access support helps people stay active, independent and connected. Whether it is attending appointments, going shopping, visiting local places or taking part in activities, the support is designed around what matters most to the individual.
Appointments
Support to attend GP, hospital, dentist, optician and other important appointments.

Help with shopping trips, collecting essentials and carrying out everyday errands.

What a Community Access Visit Can Look Like
A Community Access visit is arranged around where you want to go and what you would like to do. “A Community Access visit is arranged around where you want to go and what you would like to do. We can meet you at home, plan the outing with you and accompany you throughout the activity.”
This could involve travelling to an appointment, going shopping, visiting a café, attending a local group or simply getting out for a walk. Our support worker stays with you as agreed, providing company, reassurance and practical assistance while encouraging you to remain as independent as possible.
At the end of the activity, we can accompany you home and make sure the agreed visit has been completed.
Travel & Transport Arrangements
Community Access support can include help with planning journeys and accompanying you when travelling to appointments, shops, activities or other local destinations.
Depending on the agreed arrangement, this may include travelling with you on public transport, accompanying you in transport arranged by you or your family, or other suitable travel arrangements agreed in advance.
Any transport arrangements will be discussed before the service begins so that everyone understands how journeys will be managed.
What Community Access Does Not Include
Community access support is a non-personal care service. It does not include regulated personal care such as washing, dressing, toileting or medication administration.
